The corporate author would be the White House Task Force on Childhood Obesity. Include just the year as the date. The title of the report follows: Solving the problem of childhood obesity within a generation. Only the first word is capitalized, and it is italicized. In the example every word in the title of the report is capitalized, but that is because of the proper nouns - APA Task Force on the Sexualization of Girls is a proper noun - which are always capitalized.
