Hello!

Since it looks like there is no individual author for this report you should treat this as a report with a group author.  So the name of the group who compiled the report will be the first part of your reference entry.  Next you will want to include the date of the report in parenthesis and then the title of the report.  Finally, you will type Retrieved from and give the web address of the document.  It will look like this:

New York City Obesity Task Force. (2012). Title of the report. Retrieved from    http://www.nyc.gov/html/om/pdf/2012/otf_report.pdf

Your in-text citation will look like this:

According to the New York City Obesity Task Force (2012) the number of....

or like this:

The number of obese children has ... since last year (New York City Obesity Task Force, 2012).
